Technical Module IAPMD QAP
Proficiency testing (PT) or external quality assurance (EQA) is a process that is directly associated with the analytical outcome of the staining results. In ISO 15189, there is a special focus on external quality assessment: the Interlaboratory Comparison (ILC) – a scheme that lacks organisation on a national level in our country. Thus, the International Academy of Pathology, Malaysian Division, IAPMD, comes out with the latest product of EQA, which is the Technical Module (TM) Quality Assurance Programme. Our aims are i) to provide the external proficiency testing of laboratory methods for Hematoxylin and Eosin (H&E) staining of paraffin sections and Histochemistry stains and ii) to provide the inter-laboratory comparison exercise at least at the national level. The module covers the assessment based on the circulation of serial sections of "standard processed" tissues to be stained in many laboratories in Malaysia and evaluated by a panel of experienced pathologists and medical laboratory technologists from Malaysia.
What is unique about this TM?
The participants will use their routine slides and samples for assessment. It reflects the actual quality of the respective laboratories, i.e., their selection of slides used, their tissue preservation, their storage of the reagents, etc. That later leads to the quality of the slides for the pathologist's interpretation.
Each survey is composed of 2 exercises; i) A Hematoxylin and Eosin stained slide ( with a standardised type of tissue and the size of the tissue section) and ii) A special stain (with suitable tissue for respective stain will be advised); both are from formalin-fixed paraffin sections.
Participants are required to follow the standardised tissue type and measurements as stated in the clinical notes and submit all questionnaires (related to the respective methods used in their laboratories) via google forms.
It starts with one survey per year for the time being. The first survey is free of charge to encourage all participants to submit their material to be assessed. Then, the following year, the interested laboratories will have a chance for enrolment.
The free survey is ongoing, started on the 3rd February 2022. There are 37 registered laboratories, including two from Indonesia, eight from private laboratories, and the remaining are from government hospitals – Ministry of Health and Teaching Hospitals from public universities.
I hope many laboratories will enrol in the TMIAP QAP next year, and we will try our best to provide the standardised assessment at par with RCPA EQA. At least at the national level, we should be proud to have our own TM QAP for better anatomic pathology laboratory quality services for our country.
